Immunization requirements for next school year

Dear Parent/Guardian:

The immunization requirements for students that will be entering the 7th grand AND those entering the 12th grade in the 2017-2018 school year are as follows:

IF YOUR CHILD WILL BE ENTERING THE 7TH GRADE:

Prior to entry into the 7th grade, your child must have received:

  • One Tdap (Tetanus, diphtheria, pertussis) vaccination
  • One Meningococcal vaccination

*If your child has already received a Tdap vaccination, another one is not required for 7th grade entry*

IF YOUR CHILD WILL BE ENTERING THE 12TH GRADE

Prior to entry, your child must have received:

  • A first or second dose of meningococcal vaccine

**If the first dose of meningococcal vaccine was administered after the 16th birthday, a second dose is not required.  If the first dose of meningococcal vaccine was administered prior to the 16th birthday, a second dose is required before entering the 12th grade.

The Highland County Health Department will be providing 6th graders and 11th graders the opportunity to receive the required vaccination(s) at a school-based clinic.

The Health Department will be at your child’s school on:

Wednesday, April 5, 2017 from 8-9 at the high school and 9:30-3 at the elementary

**Please provide all insurance information requested on the consent form for billing purposes**

**If your child is not covered under Medicaid or any other insurance, the cost of the vaccine is $5.00 for each vaccine your child receives.  1vaccine = $5.oo   2 vaccines = $10.00

If you would like for your child to receive any vaccinations offered during the school-based clinic, please complete and return the consent form along with $5.00 for each vaccine the day of the clinic.  Vaccines will not be given without the signed consent form and the vaccines marked that you wish for your child to have.
